A BMI in the healthy range does not guarantee a low body fat percentage. This pattern — sometimes called "skinny fat" or, clinically, normal-weight obesity — is a real and often-missed limitation of relying on BMI alone.
Worked example
At 168 cm, 62 kg, age 55: BMI is 22, squarely in the healthy range. The Deurenberg equation estimates body fat around 34% for a woman at this BMI and age — a level some body-fat reference tables would flag as above the typical range, despite the "healthy" BMI classification.
This is an estimate, not a measurement — a direct method like a tape measurement or DEXA scan would give a more reliable individual reading. The point is structural: BMI alone cannot see this gap at all.
Why this happens
BMI only measures weight relative to height — it has no way to know whether that weight is muscle or fat. Muscle mass naturally declines with age and inactivity, a process called sarcopenia when significant, while fat mass often rises to compensate. Total weight — and therefore BMI — can stay the same even as the underlying composition shifts substantially.
What to check instead of BMI alone
- Body fat percentage — a tape measurement, smart scale, or DEXA scan gives information BMI cannot.
- Waist circumference or waist-to-height ratio — captures fat distribution, which is linked to metabolic risk independent of total weight.
- Strength and muscle mass trends — tracking strength over time is a practical proxy for whether muscle is being maintained.
None of these is perfect on its own, but combined with BMI they reveal a pattern that BMI alone is structurally unable to show.
